END-OF-LIFE PLANNING

This service is for anyone that would like to have a written plan for their end-of-life care and funeral arrangements. It includes the following:

• In-person or phone consultation with Monica Marshall to discuss your questions and wishes
• 1 draft of the written plan
• Revised final plan in digital and hardcopy form
• A copy of "Five Wishes," a legally recognized advance directive for medical care at the end-of-life (to be completed by Client)
• A "Death File" and checklist (to be completed by Client)
• One major revision of plan within 5 years of original draft

*Hourly rates may apply for plans that require research and multiple drafts at $50/hour.

This service does not constitute legal or financial advice and does not include the creation of any financial documents such as a will or trust.
